Uroczystości pogrzebowe rozpoczną się dnia 16.11.2004 roku o godz. 13.00 wyprowadzeniem z kaplicy szpitalnej na Bielanach do kościoła pw. Św. Antoniego na Wrzosach, gdzie zostanie odprawiona Msza św. żałobna. Ceremonia pogrzebowa odbędzie się na Cmentarzu Parafialnym przy ul. Wybickiego (od ul. Żwirki i Wigury).

Autokar zostanie podstawiony o godz. 12.15 przy ul. Słonecznej 35.
